Adding a jetpack contact form is causing some kind of conflict where it generates a contact form as intended but above the form standard wordpress comment fields appear also. Even when jetpack comments are activated this bug is producing the standard wordpress version.
I have tried this with various genesis themes and with the default twenty-seventeen theme. I can replicate the problem every time. It happens with posts or pages and turning off comments site wide makes no difference?
